Phoenix police: Girl wounded in apparent accidental shooting 

PHOENIX (10-30-15)  (AP) - Phoenix police say a 5-year-old girl was wounded in the leg during an apparent accidental shooting at her family's home.

Sgt. Vince Lewis says initial reports indicate the shooting occurred Friday morning after the girl slipped into the house's master bedroom while the family was having breakfast and getting ready for school.

Lewis says the gun reportedly was in a purse-type bag belonging to the girl's mother but that investigators have yet to question the girl about what she was doing.

Lewis says the bullet exited the girl's leg and that her wound is not life-threatening.

He says a police investigation continues but that something to take from the incident is that it is important to store guns in places which small children cannot access the weapons

Officer involved shooting in Bevo neighborhood; minor injuries 


ST. LOUIS, MO (KTVI) - A 42-year-old woman suffered a non-life threatening injury Thursday after being accidentally shot by police inside her south city home.

The shooting occurred in the 4400 block of Taft Avenue in the Bevo neighborhood.

According to St. Louis Police Chief Sam Dotson, the Division of Family Services were at the area with police over a one-year-old. They were investigating a fatal shooting that killed two men on Wednesday night.

Three officers went into a home with DFS personnel. Dotson said a pit bull in the home jumped over a barricade and charged into the same room as police and DFS. The officer shot at the pit bull,  missed the animal, and accidentally struck the woman in the leg.

No one else was injured. The pit bull was also unharmed.

The police-involved shooting happened in the area where two men were found shot to death at around 10:30 p.m. Wednesday. The victims were found inside a car on Taft Avenue at Ridgewood. Both men had been shot multiple times. They were pronounced dead at the scene.

MAN INJURED IN ACCIDENTAL SHOOTING AT COLLEGE OF THE REDWOODS 

(10-30-15) A man was injured late yesterday afternoon in a firearm’s accident at the College of the Redwoods Firing Range.

According to an email forwarded to us the injury “was a self-inflicted, accidental discharge and occurred during an Advanced Officer training.  The injury appears non-life-threatening; the injured officer was conscious and alert.  There is no reason for worry.”

A second email from Ron Waters, Director of the Administration of Justice school at the college describes the incident to another individual.

The person who was injured was a retired annuitant with the Humboldt County Sheriff’s Department. He suffered a flesh wound to his leg and was treated at St. Joseph Hospital. Again, there is no cause for concern on campus.

Veteran dies in accidental shooting in north Harris County 

HARRIS COUNTY, Texas (10-30-15) A veteran was killed in an accidental shooting Friday in north Harris County.

A witness told KPRC 2 the victim was loading a duffel bag into his SUV when his 12-gauge shotgun fired. A shotgun round struck the victim in his groin.

The witness, who immediately tried to help, said the victim was still conscious and told him he just returned from serving three tours of duty in Afghanistan.

The witness said the man died in his arms.

Update October 30, 2015: Yesterday, a federal judge granted a form of prosecutorial immunity to Charles Kleinert, the police detective that shot and killed Larry Jackson, Jr, an unarmed black man, in Austin, Texas in 2013.

U.S. District Court Judge Lee Yeakel ruled that because Kleinert was acting within his duties as a member of a FBI joint task force on bank robberies, he was entitled to “Supremacy Clause Immunity” based on a relatively obscure precedent set in a 1889 case.



‘He was hunted down and beaten’
The incident that led to Larry Jackson’s death on July 26, 2013 began with a prosaic chore: a trip to the bank. But that bank, Benchmark Bank, had been robbed earlier that day.

According to media and police accounts, Jackson made multiple attempts to enter the bank through doors that had been locked due to the ongoing robbery investigation. Det. Charles Kleinert approached Jackson after he was questioned by the bank’s manager. Police say Kleinert was dressed in plain clothes, but wearing his badge on his shirt. After a brief interaction, Jackson ran away.

Even though Jackson had been captured on bank surveillance video, Kleinert immediately set out alone in pursuit. Then, in a move rarely seen outside of summer blockbuster films, Kleinert commandeered a civilian vehicle. Here’s how Jordan Smith described what followed in a report from Aug. 2:

“[T]he motorist in question, sitting in a car in a parking lot near the bank, was unnerved by Kleinert’s commandeering of the car. Indeed, the source said that Kleinert was ‘out of control’ and did not effectively identify himself before directing the motorist to drive him around near the bank.”

Kleinert refused to calm down at the driver’s request, instead shouting “Go go go!” to spur the driver into action:

“When the pair drove up to a bridge that spans Shoal Creek, Kleinert spotted Jackson, who the source said was merely walking along the sidewalk. Kleinert reportedly said, ‘There he is!’ before jumping out of the car. Shaken, the motorist drove away and subsequently called the police.”

There, under the bridge bordering on the Shoal Creek Trail, Kleinert killed Jackson with a bullet to the back of the neck. This is where accounts of the incident diverge: Kleinert and Austin Police officials maintain that Jackson struggled with Kleinert and the officer’s gun accidentally fired.

This account doesn’t ring true for Adam Loewy. “First off,” he told MintPress, “Kleinert admitted under oath that Larry was not a threat, so this is unlike other cases where the cop claims the guy tried to hurt him or kill him.”

Jackson’s family and their numerous supporters believe he was deliberately murdered after a vicious beating.

Man accidentally shoots girlfriend in ear 

(10-31-15)  A 17-year-old Port Arthur man toying with a new gun accidentally fired a shot that grazed his 16-year-old girlfriend’s ear Friday morning.


The incident occurred about 10 a.m. Oct. 30 at Sunset Way Apartments, 3280 Central Mall Blvd.

Port Arthur Police Maj. Raymond Clark said the female was treated at The Medical Center of Southeast Texas.

No charges will be filed in the incident.

Meaford man hospitalized after accidental shooting  

Toronto (10-31-15) Preliminary investigation revealed that a 22-year old male was cleaning a 410 shotgun when it accidently discharged. The projectile passed through an interior wall into an adjoining room where it struck another male.

The 48-year-old victim was transported to a local hospital where he was airlifted to a Toronto hospital with non-life threatening injuries.

Grey County OPP continue to investigate the incident.

Texas DPS says FBI agent 'accidentally shot' during investigation in Crosbyton  

(10-30-15)  A Lubbock-based FBI agent was accidentally shot by a member of a state law enforcement agency Thursday evening while helping apprehend a suspect during an investigation in Crosbyton.

Mike Orndorff, an FBI special agent assigned to the Lubbock Resident Agency, was part of a group of law enforcement officials from the FBI and Texas DPS who were attempting to arrest a man when “an incident happened” about 9:30 p.m. Thursday on a dirt county road south of Crosbyton, said Texas DPS Sgt. Bryan Witt.

“The preliminary information indicates that the FBI agent was accidentally shot by a Texas Department of Public Safety Criminal Investigations Division special agent,” according to a statement from Witt. “DPS Texas Rangers, DPS CID special agents and FBI agents were all on scene as part of the investigation.”

The agencies were able to arrest the suspect, who was being held Friday at the Crosby County Jail. Information about any charges the suspect faces was not released.

Orndorff, who was a Lubbock police officer before joining the FBI, was at University Medical Center on Friday afternoon. Witt declined to provide details on Orndorff’s injuries or condition.

The DPS CID special agent who accidently shot Orndorff went on voluntary paid leave, Witt said.

FBI spokeswoman Allison Mahan said in an email to A-J Media the circumstances surrounding the shooting are under investigation, and the FBI is coordinating investigative efforts with law enforcement partners.

Witt said DPS is also coordinating efforts in the investigation into the shooting.

Man Pleads Guilty In Accidental Shooting Death of Friend  
Jesse Daniel Johnson and Logan Kelly were cleaning firearms when one of the guns fired and struck Kelly.

Cartersville, GA (10-30-15) A Bartow County resident has entered a guilty plea in connection to the 2014 accidental shooting death of a friend.

Jesse Daniel Johnson on Tuesday, Oct. 20 pleaded guilty to one count each of involuntary manslaughter and reckless conduct, Bartow County Superior Court records show.

He was sentenced under the state’s First Offender Act to 10 years, 12 months of which he will serve incarcerated and the rest on probation.

Johnson will have to abide by certain special conditions of probation, which include paying $1,197.59 in restitution to the family of victim Keypher Logan Kelly’s family and not possessing any firearms.

According to the Bartow County Sheriff’s Office, Kelly, Johnson and three other residents were at a home on Wetlands Road in White cleaning several pistols.

Johnson was in the process of putting his pistol back together when he inserted the loaded magazine into the gun, said Sgt. Jonathan Rogers.

Rogers said Johnson fired the gun after reassembly, and Kelly was hit in the upper chest or neck area. After interviews, Johnson was initially charged with reckless conduct. The involuntary manslaughter charge was added on after Kelly passed away.

Update
10/30/15  Deputies Investigating Shooting in North East Salem.
Detectives now believe the shooting took place inside of an apartment on Cheryl Lynn Way NE. One of the bullets penetrated the apartment floor and into a downstairs apartment where a father and his two children were inside, there were no injuries inside of the lower apartment.

The area surrounding the apartment is currently closed while Detectives apply for a warrant to search the upstairs apartment where the shooting took place. The male shot in the residence has since been treated and released from the Salem Hospital for a gunshot wound to the leg.


Tonight at about 8:00 p.m., Deputies were called to the 4846 Cheryl Lynn Way NE in Salem with residents reporting shots being fired. Shortly after Deputies arrived an adult male showed up at the Salem Hospital with a non-life threatening gunshot wound.

There is currently very little information available regarding the shooting. Once more information is obtained another release will follow.

Gun goes off in Altoona church during Easter Vigil mass
Police say a man accidentally shot himself in his hand at the Cathedral of the Blessed Sacrament

ALTOONA, Pa. (4-5-15)  —Police say a gun went off at about 11 p.m. Saturday at the Cathedral of the Blessed Sacrament in Altoona, Blair County.

Jason Wagler was sitting with his fiancee at Mass and heard the gunshot.    "We were standing for a few minutes and just before the final procession started, the gunshot went off," Wagler said.  Wagler said at first no one seemed to know what had happened. Some people thought a speaker blew. But then he saw smoke.  Wagler took several photos with his phone shortly after the incident happened.

"I immediately took a step back and was about to take cover. You never know if you're safe anymore in this world. It's a shame to know you can't go into church without having something like this happen," Wagler said.

Police say a man had a gun in his pocket and it discharged when he stood up. The bullet grazed the man's hand.   An Altoona newspaper reported the trigger caught on the man's pants and the gun's safety was not on.

"I noticed the gun was handed to another gentleman. He immediately concealed it in a white program, so I took pictures of the gun inside this program," Wagler said.

The man was taken to the hospital. Police haven't released his name.

It's unclear why he brought a gun into church. At this point, no charges have been filed.

FLOWOOD, MS (Mississippi News Now)  (11-25-13)  It's a place of worship that turned scary for some Pinelake Church members on Sunday morning. It's the last place some expected to hear gunfire.

"They were actually having service and were singing hymnals," said Flowood Police Detective Sgt. Billy Tucker. "Mr. (Joseph Edgar) Ray had a 9MM in his front right pocket. When they got through singing he went to sit down and the gun accidentally discharged."
Flowood police were called to the church at 8:05 am about an accidental shooting. Where Ray and his handgun were taken into custody. He was later released.

Ray has a conceal carry permit, according to Flowood police. The permit would allow him to carry a gun inside a church. However, Pinelake prohibits guns on its property.

Under state law, the church could pursue a trespassing charge if it chooses. That doesn't appear likely but Ray will still have to go to court.

"We (Flowood police) went with what we felt like was appropriate at the time, to set a precedent, said Sgt. Tucker. "It was discharging a firearm in the city limits. It goes back to safety. If you are toting a weapon you need to be safe with it. You need to have it properly secured."
Police say Ray did not have the gun in a holster.

In order to get the enhanced permit, he did have to undergo hours of training.
Investigators say Ray is a long time member of the church and was not part of the security staff.

"When you get lax, that's when accidents happen," added Tucker. "Unfortunately there was an accident but fortunately no one was injured this time."
Pinelake did issue a statement, saying in part, this is a law enforcement and legal issue.
Some believe there is no problem with carrying a weapon but they are leery of them inside a house of worship.

"I think it's fine pretty much like everyday, I mean walking around or maybe driving," said Sabrina Thornton. "But as far as a church, I don't feel as though there is a need to carry a gun into a church. That's just me." Ray's first court appearance is January 9, 2014 in Flowood Municipal Court

Accidental pistol discharge in Troy police station  
Inmate treated after bullet hits police floor

Troy (3-3-15) A Rensselaer County transport officer's pistol accidentally fired Tuesday morning inside city police headquarters when inmates were moving to and from City Court, the Rensselaer County Sheriff's Office said.
"He was attempting to put his weapon in the holster when it discharged," Sheriff Jack Mahar said.
The incident occurred at 11:25 a.m. Tuesday in an area near the police station sally port where transport officers store their weapons in lockers when they bring prisoners in to appear in Troy City Court on the second floor. The officers are not permitted to bring their weapons into the courtroom.

The bullet was fired into the floor, shattering pieces of the concrete floor, Mahar said.
An Albany County jail inmate said he was struck in the arm. Mahar said the inmate was treated at the scene by Troy firefighters and taken to Samaritan Hospital for additional examination.

The inmate, Daniel McCann, suffered an abrasion and was returned to Albany County jail where he was examined by the jail medical staff , Albany County Sheriff Craig Apple said.
The transport officer, who was not identified, was relieved from transportation duty and assigned to work inside the Rensselaer County Jail until the investigation is completed.

Pasadena Police: Officer accidentally discharges gun, colleague shot in leg  

Pasadena (10-5-15)  PASADENA >> The Pasadena Police department is conducting an internal investigation after an officer accidentally discharged a gun and shot a fellow officer in the leg Friday night, an official said Monday.

The department didn’t release the names of the two officers because it is a personnel matter.

The officer was placed on paid leave pending the result of the internal administrative investigation while the other officer is off duty due to the injury, according to Lt. Mark Goodman. He said the preliminary investigation reveals no crime occurred.

The incident happened around 8 p.m. Friday at a parking structure on Ramona Street between Garfield Avenue and Marengo Avenue.

Goodman said the two male officers were on duty at the time and were unloading items from the trunk of a police car.

He didn’t have details on how the shooting happened.

“One officer unintentionally discharged a gun,” Goodman said, adding that the round struck the other officer in the leg. He described the injury as “non-life threatening.”

The bullet went through the leg. The injured officer was taken to a hospital, treated and released, he said.

“No members of the public were present during the incident,” Goodman said.

Ocala police officer dies after accidental shooting at firearms training   
Officer Jared Forsyth dies at hospital

OCALA, Fla.  (4-7-15) - An Ocala police officer died Monday evening after he was accidentally shot during firearms training, the police department said.

Officer Jared Forsyth, 33, was shot once in the upper chest at the firing range at the Lowell Correctional Institution (see map) about 3 p.m., police said.

Forsyth was rushed to Ocala Regional Medical Center after the incident, and was taken into surgery, officials said.

"If you pray, please pray for my officer," Ocala Police Chief Greg Graham said. "He was funny. He had a true heart. I wish I had 100 like him."

Ocala police said officers were just wrapping up from the firearms course. An officer was unloading a 40 caliber Glock, semi-automatic gun when it discharged.

"The round ricocheted off a bench and struck another officer," Graham said.

Standing 25 to 30 feet away, according to Graham, the bullet entered Officer Forsyth's arm and into his chest.

Police said Forsyth flat lined several times on the way to hospital before succumbing to his injuries during surgery.

"It would be premature for me to say that we've done anything wrong necessarily," Graham said. "I'm going to do everything in my power to make sure something like this doesn't happen again."

Police have not named the second officer.

The Florida Department of Law Enforcement is investigating the incident, which is standard practice for all officer-involved shootings.

Forsyth had been with the department since April 2012.    "He will be missed by all of his OPD family, and we will keep his family and loved ones in our prayers," the police department said in a statement.

The shooting was the first fatal officer-involved shooting in Ocala in over 60 years, officials said.

POLICE OFFICER'S GUN ACCIDENTALLY DISCHARGED IN HADDON TOWNSHIP

HADDON TWP., N.J. (WPVI) -- A police officer's gun accidentally discharged Thursday in Haddon Township.

It happened around 4:45 p.m. in the unit block of Utica Avenue.   The incident involved two officers from the Audubon Police Department, police said.     Investigators said one of the officer's service weapons was discharged in the driveway of a residence.     No injuries were reported.

The Camden County Prosecutor's Office is investigating.

Accidental shooting injures 2 Idaho police officers

LEWISTON, Idaho (AP) - Lewiston, Idaho, police say a gun accidentally fired in the department's basement at the end of a training session for the force's SWAT team, injuring the hands of two of the officers.

The Lewiston Tribune reports (http://bit.ly/1W6fFbh ) the weapon fired Wednesday afternoon, and it's unclear if it happened while the gun was being cleaned or while bullets were being removed from its chamber.

Police Chief Chris Ankeny says the injuries appear to be minor.     The department has initiated an internal investigation.

VISALIA PD OFFICER'S GUN ACCIDENTALLY DISCHARGES, FAMILY MEMBER HIT  TULARE COUNTY  

FRESNO, Calif. (KFSN) -- (3-4-15) Visalia police and the Tulare County Sheriff's Office are investigating an accidental shooting by a Visalia police officer.

Police say Officer Amerie Norman accidentally discharged a personal weapon while off-duty Monday night. The bullet went through the walls of her home and hit an adult female family member. That woman is expected to be ok. Visalia police say they don't know what led up to the weapon going off.

Sgt. Damon Maurice with the Visalia Police Department said, "At this point we're not sure exactly what the circumstances were or why the weapon was out. We have asked the Tulare County Sheriff's Office to do their investigation to find those answers for us."

Officer Norman has been with the Visalia Police Department for five years. She is on paid administrative leave during the investigation.

Teen accidentally shot by off-duty officer, police say 
Incident happened at Woodridge apartment complex

RANDALLSTOWN, Md. (2-10-15)—A 14-year-old boy was shot Monday night by an off-duty Baltimore County police officer working security at an apartment complex in Randallstown, police said

The shooting was accidental, and the teen suffered non-life-threatening injuries, police said.

"There was one shot fired. He received injury to the shoulder area. He was taken to an area hospital, where he is expected to survive. Injuries are not life threatening," Baltimore County Police Cpl. John Wachter said.

Two off-duty officers were working security at the Woodridge apartment complex in the 9600 block of Southall Road shortly before 6 p.m. when a manager told them about people in a vacant apartment under construction, officials said.

"This particular building had been the scene of a previous fire and they were in the process of putting things back together," Wachter said.

The officers saw and heard movement inside the apartment, and the teen came out onto a balcony, police said. An off-duty officer told investigators he was pointing his gun toward the balcony when it accidentally went off.

The shot came from a gun privately owned by the officer and approved by the police department for him to carry. Baltimore County police said it was an accidental discharge.

Apparently the off duty officers saw movement inside the building. Police said one of them was on the front steps. He didn't have a flashlight. His hand was on his gun, but the weapon remained in its holster. The officer involved in the shooting was on a muddy hill with a flashlight in one hand and his gun in the other. Investigators said he told an on-duty officer that the weapon fired.

"He accidentally discharged his weapon when someone came out of a sliding glass door on the second floor apartment onto the balcony," Wachter said.

After being shot, police said the teen ran to the 9600 block of Button Buck Circle where a 911 call was placed from his home. He was found and taken to Johns Hopkins Hospital.

"This particular incident is very unusual. We do have accidental discharges. As part of our investigation, we're looking into exactly what happened, how fast everything happened inside of the apartment complex and what caused the accidental discharge," Wachter said.

The off-duty officer will be placed on administrative leave during the investigation, which is being conducted by the department's homicide unit, police said.

Police don't know why the 14-year-old and two other teens were inside the vacant building. Findings from their investigation into the accidental shooting will go to the state's attorney for possible charges.

Officer accidentally fires gun inside Chicago police station

Central District (2-18-14)  Prisoners at a police station in the south loop had to be transferred to another facility after an officer accidentally fired his gun inside the station.

It happened Monday night in the Central district station at 18th and State Streets.

The bullet hit the first floor ceiling, damaging a water pipe which then poured onto computers and desks.    No one was injured.

Police say the sergeant whose weapon discharged will likely face discipline within the department.

Water to the building was shut off overnight while repairs were made.

Gun Discharges, Man Injured Accidentally  (this one I'd missed)

SAVANNAH, GA (10-14-15)  Savannah-Chatham Metropolitan Police Department’s Violent Crimes detectives are working to determine how Alfonso Walker, 70, was injured after a gun discharged at a lounge on 36th Street and Waters Avenue at about 8:55 p.m. Tuesday.

Metro responded to Candler Hospital where Walker, arrived in a privately owned vehicle seeking care of a non-life-threatening injury.
The preliminary investigation shows a firearm carried by another guest at the lounge fell to the ground and discharged. The person in possession of the gun fled the area.

Investigators are working to determine whether Walker was grazed by a bullet or shrapnel. Detectives believe this incident was unintentional.

Firearm discharges on city playground; weapon belonged to off-duty officer

WEBB CITY, Mo. (3-11-15) - The Webb City Police Department is investigating the accidental discharge of a firearm involving an off-duty Webb City City police officer in a city park over the weekend.
At approximately 4:30 p.m. Saturday, officers were dispatched to the playground at King Jack Park in reference to the accidental discharge of a firearm. While at the park with his family, an off-duty Webb City police officer had to climb onto the playground equipment to help his small child who became distressed at the top of a slide. At some point, the officer lost his off-duty firearm out of his concealed carry holster.
Before he could reclaim the firearm, a 6-year old boy had picked up and discharged the firearm. The bullet struck a slide. No one was injured, according to Webb City Police Chief Carl Francis.
The Webb City Police Department’s Internal Affairs division is investigating this incident and will make a recommendation for disciplinary action

2 Abilene gun shop workers injured in accidental shooting

ABILENE, Texas (6-8-15) Two employees were shot just before 11:30 a.m. Monday at a north Abilene gun store.    Police said it appeared to be an accident.

The single gunshot went off inside the store in the 3600 block of North 6th Street.

“Somebody could have lost their life over something that could have been avoided,” said Patrick Cowen, owner of The Shootin’ Shop.     Cowen said the customer should have unloaded his .22 caliber handgun first.      “I mean it was a pretty scary experience, you know, but it was an accident,” Cowan said. “The customer feels absolutely terrible about it.”

Cowan said the customer brought the gun to the store because it wasn't working properly.   After he handed it over to an employee, the gun discharged.   Two employees were hit. One employee was hit in the hand. The other employee was hit in the abdomen.

“One shot fired that hit two people,” said Sgt. Lynn Beard, an Abilene Police Department spokesman. “Went through the hand of the person holding the gun and hit another employee standing nearby.” Those two employees were taken to Hendrick Medical Center. Police say no charges have been filed.

“We have recovered already a surveillance video of the incident,” Beard said. “It does appear to be an accident.”    The business remained closed the rest of Monday.

Cowan offered advice for gun owners.    “Unload your firearms before you take them into a building,” Cowan said. “You know, especially if you're going to have a shop look at it.”

The owner of the gun called KTXS after this story aired on KTXS News at 5.    He said he bought the gun Monday morning and loaded it in the parking lot.    He said it jammed so he put the gun back in the gun safe and the box it came in before bringing it back into the store.     He said that's when an employee opened the gun safe and accidentally fired the weapon.

(4-20-15) A Fresno police cadet was accidentally shot by another cadet Monday afternoon during the first day of weapons training at the police department’s gun range.

Fresno police Chief Jerry Dyer said the accident happened shortly before 5 p.m. as cadets were finishing the day’s training with the Fresno City College Police Academy at the gun range southwest of Fresno, near West Hayes and South Central avenues and south of the city’s wastewater treatment plant.

Cadets were cleaning their guns when one cadet, whose identity was not released, accidentally fired a round at the victim’s pelvic area. The cadet who fired the gun is not affiliated with any law enforcement agency, Dyer said.

The victim, who Dyer described as “sharp” and in his early 20s, was transported to Community Regional Medical Center. The cadet is in stable condition and was awaiting X-rays Monday evening to see if surgery was needed.

Dyer said the gun range is operated and organized by the State Center Community College District and features cadets who are either going through the training as employees of various law enforcement agencies or through the college district in hopes of landing a job afterward.

Monday marked the first time this year’s cadets, who enrolled in the training program in January, practiced using firearms. The program, Dyer said, lasts through June. Seven cadets have already been hired by the Fresno Police Department.

The victim has been with the Fresno Police Department for about a year. The gun discharge is believed to be accidental, Dyer said, but the department and the college district will conduct a joint investigation. The cadet who fired the weapon will be disciplined by the college district.

Dyer said incidents like these are rare: Monday marked the first accidental shooting involving a Fresno police employee in 35 years, he said.

“We’re very fortunate today there weren’t any serious injuries,” Dyer said. “It’s unfortunate that we had an accidental discharge with a cadet, but we’re very fortunate that the round did not cause any more of an injury than it did.”

Investigation Underway Following Accidental Discharge of Officer’s Gun in Pacoima  

(5-6-15) Authorities were investigating the accidental discharge of an officer’s gun in Pacoima Wednesday morning.

The incident took place near the intersection of Van Nuys Boulevard and De Foe Avenue (map), the Los Angeles Police Department’s media relations department confirmed.

Aerial video from Sky5 showed at least six patrol cars parked in front of a home on Van Nuys Boulevard just before 8 a.m.

A man who appeared to be in handcuffs was placed into the back of one of the patrol vehicles, the video showed.     It was unclear why officers were initially called to the location but at some point during the investigation an officer’s weapon was accidentally discharged, according media relations.

Authorities did not say what caused the weapon to be discharged, but the incident was under investigation.    There was no word on whether anyone was injured when the gun fired.

Aerial video did not show any emergency vehicles at the location.

Officer shot by accidental discharge, taken to hospital 

WESTTOWN Pa. (2-2-15)  An off-duty police officer was rushed to Paoli Hospital on Monday after he suffered a gunshot wound to the leg in an apparent accidental discharge.

According to Westtown-East Goshen Regional Police Department Lt. William Cahill, the off-duty officer was preparing a trip to a local firing range shortly before 2 p.m. when a firearm accidently discharged at an unknown location away from the police station. The fired round struck the officer in the leg, and he then drove himself to the police station for help, Cahill said.

Emergency medical workers quickly responded to the police station, located in the 1000 block of Wilmington Pike, and rushed the officer to Paoli Hospital.

The identity of the officer was not released. His condition was not immediately known but the injuries did not appear life-threatening.

“Hopefully everyone is going to be just fine,” Cahill said, adding that the department will conduct an investigation of the shooting.

The Good Fellowship Ambulance Company and Chester County Medic 91 assisted at the scene.

Florence police officer accidentally fires gun at police car during training 

(10-24-14) Fri, 24 Oct 2014 23:10:42 GMT — Florence Police Chief Allen Heidler says he's conducting internal investigation into the accidental discharge of a firearm during a training exercise last Tuesday off of Tunnell Road in Darlington County.

Heidler said his lieutenant, who is a certified instructor through the South Carolina Criminal Justice Academy, mistook his real gun for a training weapon, which is a fake gun.

Heidler released the following statement to WPDE about this incident:

"Last Tuesday, October 14th, while in training with the Darlington County Sheriffs office, off of Tunnel Road in Darlington County, an officer of the Florence Police Department accidentally discharged his duty firearm. The officer's firearm was aimed away from other training participants however the bullet struck the windshield of an unoccupied Florence Police vehicle. It appears that the officer believed he was in possession of a training firearm at the time. The accidental discharge as well as the failure to follow appropriate training safety protocols is under safety and disciplinary review," said Heilder.

WPDE questioned Heidler more about how the officer could have made the mistake.

"It shouldn't have been mistaken, It is a safety protocol failure. The structure and operation are generally the same but have coloring to differentiate them from a firearm," Heidler explained in an email.

Major Florence McCants with the SC Criminal Justice Academy said they use colored training weapons to make sure no one confuses the weapons. The Academy trains hundreds of law enforcements every year on firearm safety.

"We actually have training weapons just for training. So, when you have defensive training what we have is a weapon, the color is all blue. And when we're here on the range, we're doing the range type training, those weapons are all red. So , you know that those weapons aren't real active weapons," said Major McCants.

McCants added it's always important to follow all safety measures and protocols during training exercises.

McCants explained, "When you have a handgun in your hand, you treat it as if it's a live round whether or not it is, you do it as such. And if you do, you're less likely to have those types of incidents."

Heidler said they are fully looking into their safety protocols and whether the safety failures that occured the day of the accident extend beyond one officer. He added it will be an extensive review and will take some time.

Five hurt after gun accidentally discharges in Clearwater Beach hotel lobby 

CLEARWATER (5-14-15)— Five people, including two children, sustained minor leg injuries Thursday afternoon after a gun was accidentally fired in the lobby of the Clearwater Beach Marriott Suites on Sand Key, police said.

Officers responded to the hotel at 1201 Gulf Blvd. about 2:30 p.m. after receiving reports that a hotel guest carrying a 9mm Glock reached into the pocket of his shorts and accidentally discharged the weapon.

Stan Lee, 46, of Ormond Beach, had been grabbing for his wallet and instead grabbed the gun, police said. The bullet grazed his leg and then hit the floor, scattering fragments that struck a hotel employee, another hotel guest, and two children ages 5 and 2.

Two people, including the gun owner, were taken to Morton Plant Hospital with minor injuries. The others sustained superficial wounds and were treated at a local emergency clinic, Clearwater police said.

"A charge of culpable negligence was forwarded to the Pasco-Pinellas State Attorney's Office," police said.

FMPD: 2-year-old hurt in "accidental" shooting  

(10-29-15)  A 2-year-old is being treated at Gulf Coast Medical Center after suffering what the Fort Myers Police Department called an "accidental gunshot wound."

Police said a call was received from the hospital Thursday morning alerting them to the child's injuries, which FMPD described as "non-life-threatening."

A family member said the child was hit in the thumb.

The shooting occurred in Fort Myers and the case is being investigated, police said.

Hudson man injured in apparent accidental shooting

HUDSON - (10-29-15)  Pasco County Sheriff's Office deputies are investigating an apparent accidental shooting in Hudson.

According to the victim, he and his girlfriend were inside their home on the 12000 block of Kitten Trail when they began to argue.

The victim, identified as 21-year-old Michael Stroehlein-Toro, exited the home and went outside. While outside, he fired a shotgun in the air, injuring his neck, deputies said. 

Stroehlein-Toro, who was alert, talking and never lost consciousness, was taken to Regional Medical Center Bayonet Point for treatment.

Deputies are still investigating but said there is no indication of foul play or suicide attempt at this time.

North Fort Myers man accidentally shoots himself  

(10-19-15)  Deputies responded to what was reported as a shooting Wednesday afternoon on Marx Dr. in North Fort Myers.

It ended up being a man who shot his own arm by accident and lied about.it to deputies, the Lee County Sheriff's Office said.

Neighbors in the North Fort Myers community of Suncoast Estates said they heard a gunshot about 3:20 p.m. The sheriff's office helicopter circling the area until about 3:35 p.m.

The sheriff's office sent at least three deputies, a K-9 unit and had a helicopter circling above.

Stray bullet hits 6-year-old girl; man charged 

 New Castle  (10-29-15)  A New Castle-area man with an extensive criminal record was arrested and charged Wednesday in the accidental shooting of a 6-year-old girl early this month in Wilmington.

Court records obtained by The News Journal show that Korie D. Henry, 31, was firing at another black man from the corner of Taylor and Spruce streets when a stray bullet from his gun hit the 6-year-old girl, causing her to fall to the ground. The girl, identified by her mother as Armoni Fields, suffered a shattered right kneecap and a broken leg.

Those that live in the neighborhood said the young girl was best known for wheeling her dolls up and down Bennett Street in a baby stroller, just steps from where she lives.

Community cooperation and investigation were credited in the arrest of Henry, now being held for lack of $410,000 cash bail on charges from the Oct. 5 shooting, city police said Thursday.

He was charged with first-degree assault for intentional reckless and serious injury during commission of felony and firearm possession during commission of a felony, said Sgt. Andrea Janvier, public information officer for the Wilmington Police Department.

Henry also was charged with firearm possession by a prohibited person, following a prior conviction for a violent felony, she said.

Officers on routine patrol at about 6:40 p.m. Oct. 5 alerted headquarters after hearing gunfire in the area of the 800 block of Bennett St., then gave the girl first aid and took her to Wilmington Hospital, Janvier said.

Firing of tank gun led to explosion that killed 2 Ore. City men

BEND, Ore. - (10-29-15)  Two Oregon City men died after an explosion inside a World War II-era tank at a firing range east of Bend Tuesday, police said.



“This is a tragic day," said Sheriff L. Shane Nelson. "Two families lost loved ones, and our hearts and prayers go out to them."

Austin Lee, 22, and Steven Preston, 51, were helping with a film project featuring the tank when the accident happened around 3 p.m.

"The film crew was capturing video of the tank destroyer firing rounds," said Sheriff Nelson. "The video was to be used as part of a display of the tank destroyer at a future exhibit."  He added that the explosion occurred inside the tank and was a direct result of firing the tank destroyer's gun. The exact cause of the explosion is still under investigation.

Paramedics responded quickly and tried to save the two men, but they died at the scene, state police said.

The film crew was working at a Deschutes County firing range operated by the Central Oregon Shooting Sports Association, near the town of Millican, about 25 miles southeast of Bend.

Preston's Facebook page shows a photo of what appears to be the tank, surrounded by a smokey haze created by a fog machine. Other posts show the tank on display in various locations, surrounded by flags.

Rock Hill 2-year-old finds loaded gun, shoots grandmother in back

(10-12-15)    A 2-year-old Rock Hill boy accidentally shot his grandmother, police say, after he found a loaded .357 revolver in the car they were riding in and pulled the trigger.


The shooting happened just after 1 p.m. Sunday as the car passed through the intersection of Ogden Road and Heckle Boulevard in southern Rock Hill, police records show. The 40-year-old victim told officers she heard the gun go off and realized she had been shot in the back.

When the boy’s great-aunt, who was driving the car, turned around to see what had happened, she told police, the boy “had the gun in both hands and began to cry.”

Police initially said the boy was 4, but later learned he is 2. The mistake happened due to “confusion at the scene” on Sunday, a spokesman said.

Capt. Mark Bollinger, spokesman for the Rock Hill Police Department, said the great-aunt is the legal owner of the gun.

Before the shooting, the boy’s relatives had picked him up from another family member’s home, according to police. The gun had been left in a pouch on the back of the car’s passenger seat.

“We’re still trying to figure out how the child pulled the trigger,” Bollinger said, adding that the boy was wearing a seat belt but was not in a carseat


After the shooting, the police report states, the boy’s great-aunt drove the car to a family member’s home on nearby Stanley Drive. The boy’s grandmother was treated at Carolinas Medical Center in Charlotte with non-life threatening injuries, Bollinger said.

11-year-old boy accidentally shot by teen brother during family quail hunt

(10-12-15) An 11-year-old boy was wounded this weekend when his older brother accidentally shot him during a family quail hunt in Tuscaloosa.

Authorities were called to the 13000 block of North Rosser Road about 4:30 p.m. Saturday on a reported accidental shooting. Police said a 69-year-old man had taken his two grandsons, ages 16 and 11, quail hunting on their property, said Tuscaloosa County Metro Homicide Capt. Gary Hood.

The 16-year-old, Hood said, accidentally shot his younger brother while trying to shoot at a quail. The boy was struck by birdshot, and received injuries to his face and left hand. The boy was taken to DCH Regional Medical Center. His injuries aren't life-threatening, Hood said.

Authorities didn't release the names of those involved.

MBPD: Accidental hotel shooting sends woman to hospital   

(10-12-15)   MYRTLE BEACH, SC (WMBF) - An accidental shooting at a Myrtle Beach hotel sent one woman to the hospital Monday morning, according to the Myrtle Beach police.

Officers said the shooting happened early Monday at Hotel Blue. A female suffered a gun shot wound to her leg. Police believe the gun fell and went off on her inside a hotel room.

The woman was taken to the hospital and was alert and believed to be alright

Cops: Babysitter facing charges after shooting hand 


BRIDGEWATER — (10-12-15) Police say a central Vermont man has been charged after he accidentally shot himself while babysitting a pair of children.

Vermont State Police are investigating after the 26-year-old babysitter discharged the weapon Saturday night near the children — ages 1 and 2 — in Bridgewater, located 17 miles east of Rutland.

Troopers responded to a residence on U.S. Route 4 after a report of a firearm discharge with injury.

An investigation revealed that the shooting was accidental and the round hit him in one of his hands, ricocheted off the floor, and flew outside through a sliding door.

The children were upstairs sleeping and were not injured.

The man was charged with reckless endangerment and is due in Windsor County Superior Court on Dec. 22.

Retired Mac teacher in hospital after accidentally shooting himself 

(10-12-15)  BUNKER HILL – A retired teacher at Maconaquah Middle School remains hospitalized after deputies say he accidentally shot himself in the abdomen with a rifle.
Witnesses told deputies that Jim Jenkins was attempting to stop two dogs from fighting with a .22 caliber rifle when the gun discharged and hit him in the abdomen.
Deputies said Jenkins fired a few shots in the air in an attempt to stop the dogs from fighting. He then struck one of the dogs with the butt of the gun, which caused it to discharge.
Jenkins was air lifted to Lutheran Hospital in Fort Wayne, where he underwent surgery for his wounds, deputies said.
He remained hospitalized Monday in critical condition, according to hospital officials

(10-28-15)    ACWORTH, Ga. -- Authorities said a 2-year-old boy accidentally shot and killed himself at a home in Acworth Tuesday evening.
Police said it happened in the 1000 block of Athena Court in a subdivision near Interstate 75.

According to Acworth Police Officer Youlanda Leverette, the 2-year-old was able to get ahold of a family handgun that was left on a bed when the boy's father stepped away Tuesday evening.  The child got the gun and shot himself in the head.

Authorities said the child's father was home, the mother was at work.
Leverette said no charges had been filed as of this point, calling it a "tragic accident. A preliminary investigation is underway.

Two more arrests made in fatal accidental shooting

SIOUX FALLS - (10-28-15)  Two more arrests have been made after an accidental shooting in northwest Sioux Falls left one person dead.

According to Public Information Officer Sam Clemens, 19-year-old Desmond Juan Henderson and 19-year-old David Alexsander Ramirez are both facing charges for their role in the incident.

Authorities say a group of people were drinking and smoking marijuana while passing around a loaded AK-47 at an apartment on W. Briggs Drive. The gun went off, killing 19-year-old Mikael Ashame.

Clemens says after the shooting, Henderson grabbed the rifle and put it in Ramirez's van. Ramirez drove off, and allegedly threw the weapon into an area near 12th Street with some trees.

Two guns were found at 12th Street and Ribbon Place by Skunk Creek, Clemens says. One was an AK-47, the other was a .22 rifle.

Both Henderson and Ramirez were arrested Tuesday evening.

Henderson is charged with accessory to conceal physical evidence, possession of a firearm by a convicted drug offender, and various drug possession charges stemming from items in a backpack he had. He is being held on a $75,000 cash bond. Ramirez is charged with accessory to conceal physical evidence. He is being held on a $50,000 cash bond.

Police arrested other suspects in the incident shortly after the incident happened. Police say David Randle Jr. was holding the gun as it went off. He is charged with first-degree manslaughter.

The owner of the gun, Mason Mitzel, is also facing various drug-related charges.

Man Injured in Self-Inflicted Accidental Shooting

(10-28-15)  A North Star Township man was injured Wednesday morning after accidentally shooting himself in the leg.

Authorities responded to Northwoods Road and Pequaywan Lake Road in North Star Township around 9:30 on reports of an accidental gun shot wound.

They say 63-year-old Clark Coole was road hunting in his car when he saw a grouse and loaded his shotgun while exiting the vehicle.

Coole slipped due to wet road conditions and the gun fired into his leg.

He was transported by ambulance to St. Lukes Hospital in Duluth with non-life threatening injuries.

The incident remains under investigation by the St. Louis County Sheriff's Office.

Thomasville shooting determined as accidental


THOMASVILLE — (10-28-15) Deputies said a 16-year-old boy faces criminal charges following the investigation of an "accidental shooting."

William Michael Conrady, 16, is charged with breaking and entering of a motor vehicle and larceny of a firearm, a news release from the Davidson County Sheriff's Office stated.    Deputies filed the charges following an investigation of what they determined to be an accidental shooting of an 18-year-old woman.

The shooting happened on Oct. 14 in the 100-block of Kimberly Lane in Thomasville.
Investigators determined the gun Conrady allegedly took was the same one used to shoot the woman, the release said.       Deputies said Conrady faces other pending charges in a case unrelated to the shooting.

Man who shot self, wounded 9-year-old neighbor, arrested

(10-28-15)  SAN JOSE, Calif. (KTVU and wires) - Authorities have arrested the man who shot himself in the foot with a hunting rifle before the bullet passed through the floor, injuring a 9-year-old boy in San Jose early Wednesday morning, police said.

40-year-old San Jose resident Eric Smith will be arrested on suspicion of negligent discharge of a firearm, being a felon in possession of a firearm and being a felon in possession of ammunition, police Sgt. Enrique Garcia said.

Around 5:45 a.m., officers responded to a report of someone shot in the 1900 block of Almaden Road, Garcia said.


Arriving officers found the upstairs neighbor, later identified as Smith, and the child, according to Garcia. The bullet struck Smith in the foot before passing through the floor, striking the boy in his leg as he slept.

Smith and the boy were both taken to a hospital to be treated for gunshot wounds that are considered serious but not life-threatening, Garcia said.

Smith allegedly used a 30.06 rifle, according to Garcia.

People who live at the Almaden Apartments were in disbelief. Martin Camberos lives next door to the suspect and victim's apartments.

“I just remember people screaming a lot,” said Camberos. “I heard the mother crying because she was taking her kid to the hospital because he was bleeding a lot."

North Fort Myers man accidentally shoots arm  

(10-29-15) Deputies responded to what was reported as a shooting Wednesday afternoon in North Fort Myers.

It ended up being a man who shot his own arm by accident and lied about.it to deputies, the Lee County Sheriff's Office said.

Neighbors in the North Fort Myers mobile home community said they heard a gunshot about 3:20 p.m. The sheriff's office helicopter circling the area until about 3:35 p.m.

The sheriff's office sent at least three deputies, a K-9 unit and had a helicopter circling above. Less than an hour prior, Fort Myers police responded to a fatal shooting.

Police: Teen accidentally shot in face while posing with gun 

(WBIR-CHATTANOOGA)  (10-28-15) A Chattanooga teen is recovering after he was shot in the face while posing for a picture with a gun.

The shooting happened Sunday night at a home on Buck Trail in Harrison, according to WRCB.

Neighbors heard the gunshot, and one woman said she heard a woman say "Where is he shot at? Where is he shot at?"

Investigators say it appears a family member accidentally fired the gun while they were posing for pictures with "illegally obtained" weapons.

The 13-year-old boy is expected to fully recover.

No charges have been filed, but the investigation is still open.

CLE police: Father taken into custody after 4-year-old boy shot in shoulder

(10-28-15)  CLEVELAND– Cleveland police say they responded to Bliss Ave. around 5:30 p.m. Wednesday for a 4-year-old boy who had been shot in the shoulder.

At first, the boy’s father told police it happened during a drive-by shooting, then said a 5-year-old accidentally shot the child.

Police determined that the shot was fired from inside the home. They initially said the father may have accidentally shot the 4-year-old, but Cleveland Police Chief Calvin Williams says they are still investigating who fired the shot. The child’s father was taken into custody for endangering children and weapons charges.

The 4-year-old was taken to Rainbow Babies and Children’s Hospital. Police say at this time, the child should not need surgery. The boy’s mother is with him. Police say there were 4 other children in the home; they are in the custody of Children and Family Services.

The investigation continues. We will bring you any new details as they come in.

2-year-old boy dead after accidentally shooting himself in Acworth home 

ACWORTH, GA (10-27-15) -- Authorities said a 2-year-old boy accidentally shot and killed himself at a home in Acworth Tuesday evening.

Police said it happened in the 1000 block of Athena Court in a subdivision near Interstate 75.

According to Acworth Police Officer Youlanda Leverette, the 2-year-old was able to get ahold of a family handgun that was left on a bed when the boy's father stepped away Tuesday evening.

Leverette said no charges had been filed as of this point, but that the preliminary investigation was underway.

She said there was also a 4-year-old boy in the home at the time of the incident. The boys' mother also lives in the home, but was not at home when the accident took place.

Leverette said police were dispatched to the home just before 7:30 p.m.

She said this was a sobering reminder to everyone to put weapons away, to use gun locks and to keep weapons out of the reach of children.

One man in critical condition after accidental shooting  

BUFFALO, N.Y. -- (10-27-15)  Buffalo Police are investigating an accidental shooting that occurred on Monday evening.

Police say the shooting happened around 6:15 p.m. Monday evening in the 100 block of Thatcher Avenue.

One 18-year-old man suffered an apparent accidental gunshot wound.

He was taken to ECMC where he is listed as in critical condition.

Police have not released the name of the victim at this time.

Two women injured in accidental shooting at Targetmaster gun range  


MIDDLETOWN (10-27-15)   A 60-year-old West Chester woman accidentally discharged her firearm during an introductory handgun safety course at Targetmaster, wounding herself and a 44-year-old Delaware woman, according to Pennsylvania State Police.

The two women were students in the class, and the accident occurred during the live-fire portion of the course, shortly before 9:30 p.m. Monday.

The bullet discharged from the West Chester woman’s firearm struck both women, according to State police spokesman Trooper Timothy Greene. The Delaware woman was not handling a firearm at the time of the incident.

The West Chester woman was taken to Chester County Hospital with an arm wound, while the Delaware woman was taken to Crozer-Chester Medical Center with a wound to her torso area. Their conditions were not immediately available.

No charges are expected to be filed, according to Greene.     The incident was investigated by Trooper Jonathan Pipes of the Media Station in Middletown.

Targetmaster advertises itself as the premier indoor firearm range and gun shop in the Delaware Valley on its website. It is located at 255 Wilmington West Chester Pike in Chadds Ford.

Buffalo teen in critical condition after accidental shooting

(10-27-15)  A Buffalo teenager was in critical condition Tuesday after suffering what police believe was an accidental gunshot wound, police said.

The 18-year-old was shot at around 6:15 p.m. Monday on Thatcher Avenue, between East Amherst Street and Kensington Avenue. Detectives said it appeared to be an accidental shooting.

The man was taken to Erie County Medical Center where he was listed in critical condition.

Duck hunter in kayak accidentally shoots himself, airlifted to hospital 

(10-27-15) BARRY COUNTY, MI -- A Shelbyville man suffered a severe gunshot wound to the arm after he accidentally shot himself while exiting a kayak and entering a duck blind along Otis Lake on Tuesday.

Police were called just after 8 a.m. Oct. 27 to the Rutland Township lake after two other men who were duck hunting witnessed the shot fired and called 911.

Michigan State Police troopers responded, and using a Barry County sheriff's marine unit boat, were able to reach the injured man. He was taken to a waiting AeroMed helicopter and transported to Grand Rapids for treatment of his injuries, police say.

Man injured in accidental shooting in Ozark County  

(10-27-15)  A hunting accident in Ozark County over the weekend resulted in a man being shot in the foot with a muzzleloader, according to the Ozark County Times.
    The paper reported the 50-year-old man shot himself in the foot on Saturday morning with a black powder rifle during the muzzleloader management hunt at the Caney Mountain Conservation Area, north of Gainesville.
    The unidentified man reportedly told authorities that his muzzleloader had discharged and the bullet went through his foot. The patient reportedly refused medical treatment at the scene.

Lakewood police officer accidentally shot at firing range

DENVER (AP) — Authorities say a Lakewood police officer was shot in the leg at a Jefferson County law enforcement firing range and training facility.

The Denver Post reports (http://goo.gl/ivDmPs ) the officer, whose name has not been released, was hit by a round that ricocheted Tuesday morning. Her injuries were not life-threatening.

The officer, who was at the training area's pistol range, is believed to have been struck by a round fired at a nearby rifle firing range.

Accidental shooting kills local hunter


(10-28-15)  A Greenville barber and avid hunter was killed in an accidental shooting while in his tree stand, authorities reported on Tuesday.

Melvin “Mel” Boyd Jr., 57, of Greenville, was shot sometime before 7 p.m. Monday in a wooded area near the Pitt-Greene county line. His family owns Boyd’s Barber Shop on Evans Street near downtown Greenville, and he worked there as a barber.

The medical examiner will determine the exact cause of death, but it appears he accidentally shot himself, Gary Harrison, a wildlife enforcement officer in Pitt County said. The single gunshot wound was to Boyd’s head, he said.

“He was in the hunting position in the tree stand,” Harrison said. The stand was located about two miles from the 8900 block of Stantonsburg Road northwest of Farmville.

Boyd was hunting with a 14-year-old boy who he was mentoring, and the two went into the woods together, according to Harrison. The boy was in one tree stand that was not visible and was out of shouting distance from the tree stand where Boyd set up to hunt, Harrison said.

At some point, the boy tried to communicate electronically with Boyd and was not able to reach him, Harrison said.

The Pitt County Sheriff’s Office originally received a call about 7 p.m. saying Boyd was missing, but before deputies arrived, Boyd’s body was located in the tree stand.

There did not seem to be any indication that the boy or any other hunter may have accidentally shot Boyd, Harrison said.

“It doesn’t appear that anybody was involved with the discharge other than the hunter,” Harrison said.

Harrison and Pitt County deputies went to the scene, took photos and discussed among themselves how they believe the accident happened, but it will be up to the Medical Examiner’s Office to make the final determination, he said.

Harrison said there are several ways a hunter can accidentally shoot himself. One may be that if a rifle has a mechanical failure, it could accidentally discharge as the hunter is trying to determine why or trying to fix it, he said.

Harrison said fatal hunting accidents are rare, and they often happen as people are climbing in and out of a tree stand. Hunters are sometimes injured or killed when they fall out of a tree stand, so Harrison recommends hunters use a fall restraint system

“We recommend they utilize safe firearm arm handling procedures,” Harrison said. “If you’re not sure what those are, we recommend a hunter safety course to learn those techniques.”

The basic rules of firearm safety are to keep the firearm arm unloaded except when ready to shoot; always point the firearm in a safe direction; keep the safety on except when firing a shot; keep the firearm unloaded when getting in or out of a tree stand; and use a safety line to move the unloaded firearm up and down in the tree stand.

Harrison urges hunters or anyone who wishes to begin hunting to take a six-hour course offered free by the N.C. Wildlife Resources Commission. The next course is scheduled to begin Nov. 16 at 6:30 p.m. at River Park North at 1000 Mumford Road in Greenville. Registration is required.    Every county in North Carolina offers the free courses.

Victim died within minutes of accidental shooting  

(10-26-15)  Sioux Falls -   Two other men were arrested following the incident: one on drug charges and one for manslaughter.

Sioux Falls Police arrested the owner of a gun involved in a fatal, accidental shooting in a Sioux Falls apartment.

Mason Mitzel, 19, faces several drug-related charges after authorities found $30,000 worth of drugs in his car, police said.

He's accused of bringing an AK-47 rifle to the apartment, where it discharged early Saturday, killing 19-year-old Mikael Ashame.

David Randle, Jr., 21, was holding the gun at the time it went off and is charged with one count of first-degree manslaughter.

The bullet struck Ashame in his leg, hitting an artery. He died within minutes, police said.

Police investigating Chesterfield father's death as an accidental shooting

(10-26-15)  With his wife visiting relatives in Delaware, Nick Clavier and his four young children planned to spend Saturday afternoon fishing, a favorite family pastime. The five had stopped for candy and were en route to pick up some worms when an inexplicable tragedy struck.

Clavier suddenly veered off the road and crashed about 1:15 p.m. in woods in the 8700 block of Hull Street Road in Chesterfield County, police said. His children, ages 9, 8, 6 and 4, escaped harm.
Clavier suffered serious, life-threatening injuries, but apparently not from the crash. Responding officers discovered he had been shot.

The 32-year-old lifelong Chesterfield resident was taken to Chippenham Hospital, where he died.
Grieving family members are still trying to sort out what happened.

“All we know is that a gun went off — either his gun or somebody shot him, one or the other,” said Clavier’s father-in-law, the Rev. Nathan Boehm. Clavier had a concealed-weapon permit and would always carry his firearm on trips with his children, Boehm said.

Police don’t suspect foul play and are investigating Clavier’s death as a possible accident.
After crashing in the woods, Boehm said his 9-year-old granddaughter took command of her siblings and led them to safety at a nearby church.

“Right away, she opened the door and got the kids out, which was quite a feat for several reasons,” said Boehm, pastor of the Richmond Family Worship Center at 7424 Belmont Road.
For one, Boehm said, the eldest had to coax her younger sister, 8, who has Asperger’s syndrome, out from under one of the car seats where she was hiding.

Barefooted, the eldest child then led her younger sister and two younger brothers “through a lot of brush that was as tall as she was” through the woods and to a nearby church. “But she still did it because she was trying to save the family,” Boehm said.

When family members on Sunday revealed to the children that their father had died, the eldest girl “started to take the blame” for the tragedy, Boehm said.

“I tried to save my family, I tried to save them,” Boehm said the girl plaintively cried.   “Just 9 years old,” Boehm noted. “I’m really proud of her.”

Boehm said his son-in-law occasionally took his children fishing on weekends and they had stopped at Clavier’s mother’s house before heading out.     They then apparently stopped for candy and were “heading to get some worms” when the crash occurred, Boehm said.

Clavier’s wife of 11 years, Melody, was driving to Delaware with Boehm’s wife and youngest daughter to attend a family member’s retirement party when they got the news, Boehm said.
“They got there and five minutes later had to turn around and head back,” Boehm said. “It took them five hours to get back because my daughter was sick ... with nerves. So my guess is that was probably the longest five hours she’s ever experienced.”

Community members have established a page for the Clavier family on the GoFundMe website with a goal of raising $100,000. About $17,500 had been raised as of 5:30 p.m. Monday.
“The CLAVIER Family is in desperate need of PRAYER first and foremost!” the page narrative reads. “Their family was struck by tragedy when the patriarch, Nick, suffered a gunshot wound to the head, on his way to go fishing with his four kids.”

UPDATE: Mason City shooting called “accidental”

MASON CITY, Iowa – (10-26-15)  Authorities say a man shot himself in the abdomen in Mason City.

The incident happened around 11:57 pm Sunday.  Police officers responded to the 300 block of 9th Street SE for what is being described as an accidental shooting.

The man, who has not been identified, was transported to Mercy Medical Center – North Iowa for treatment.  An investigation is continuing

 MASON CITY, Iowa – A Mason City man sought emergency treatment after suffering a gunshot wound to his abdomen. Police responded to the 300 block of 9th street southeast overnight Sunday, on a report  a man needed emergency assistance after being shot.
The circumstances are unknown at this time although, it is not thought to be a suicide attempt.
Mason City Police are investigating.

Accidental gunfire injures officer at Belleville police headquarters  

Belleville N.J.  (10-26-15)  A Belleville police officer suffered a gunshot wound after his gun accidently fired inside the police station.
According to the Essex County Prosecutor, the gun belonged to the officer and went off inside the detective bureau at around 3:30 p.m. Monday. The officer suffered a flesh wound to his leg.
He was taken to a local hospital to be treated and is expected to be OK. No one else...

Teen killed in hunting accident, coroner says

NEWBERRY COUNTY, S.C (10-26-15)  A teenager was killed in a hunting accident Sunday in Newberry County, the coroner said.


Newberry County Coroner Craig Newton said 18-year-old Kendall Lesher Williams of Prosperity was killed at about 7 p.m. Sunday.  The accidental shooting happened off Highway 391 near Big Creek Road in Prosperity, Newton said.

Newton said Williams suffered a single gunshot wound to the abdomen.  The circumstances of the shooting were not released.

Williams was a student at Mid Carolina High School and was a senior.

The death is being investigated by the Department of Natural Resources Law Enforcement Division and the Newberry Coroner's Office with assistance from the Sheriff’s Office.

Newton said Williams’ death has affected several families, and he has asked that their privacy be respected.

"I offer my prayers and sympathy to all who have been touched or affected by this tragic event. My heartfelt condolences are with you all during this difficult time," Newton said.

Principal Ray Cooper released a statement that says: “It is with heavy hearts, that we at Mid-Carolina High School mourn the loss of one of our own, Kendall Williams who passed away this weekend.  Kendall Williams was an outstanding young man who was liked by all and he will be greatly missed by students, and staff alike.

“It is a sad and difficult time at MCHS for students and staff as we cope with this tragic loss.  Additional support is being provided for any student or staff member who needs it.  In addition to our own school based counselors, all of our surrounding elementary, middle, and high schools have sent their counselors to MCHS to ensure that we have enough help available.

“Westview Behavioral Center has also provided a clinical counselors as well.  Support zones have been established in multiple locations across our campus with a counselor assigned to each zone.  Also, at the request of the students, the Media Center is being made available in the mornings, and during lunch as a quiet place for students to meet and collectively mourn this tragic loss.”

13-year-old shot at military base has died; friend charged  

The 13-year-old boy shot in the face last week by another teen at Joint Base Lewis-McChord has died.

(10-26-15) JOINT BASE LEWIS-MCCHORD, Wash. (AP) — A 13-year-old boy who was shot in the face last week at a military base in Washington state has died, and his 17-year-old friend remains in juvenile detention, police said Monday.

The boy died Sunday night at a hospital, where he was taken after being shot last Tuesday at Joint Base Lewis-McChord, said Lakewood Police Lt. John Unfred.

Unfred told The Associated Press the boy sneaked onto a remote, wooded part of the base with his 14-year-old brother and their friend, apparently so they could “shoot off a few rounds” from an old handgun they had somehow obtained. They entered the base through a hole that had been cut in a perimeter fence that abuts a neighborhood, he said.

The victim’s brother told authorities that the three boys were walking in the woods when he heard a gunshot; he turned to see his brother lying on the ground and the 17-year-old holding a gun, court records said.

The older boy, who ran off after the shooting, later said it was an accident, The News Tribune newspaper of Tacoma reported (http://is.gd/ZAzvPs ), citing court records. He pleaded not guilty Friday to charges of third-degree assault and unlawful possession of a firearm, charges which had not been changed as of Monday morning.

Lakewood police are investigating how the boys got the gun, Unfred said. The U.S. Bureau of Alcohol, Tobacco and Firearms ran a trace on the weapon, which showed the last official documentation of it to have been a sale in Kansas about 35 years ago, he said.

Two-Year Old Girl Accidentally Shot, Killed In Henderson County

(10-26-15)    A two-year old girl was accidentally shot and killed Monday morning in Henderson County.

The Henderson County Sheriff’s Office tells us the shooting was reported at 9:02 a.m. after Abigail Newman was shot by a shotgun. The young girl was taken to Mission Hospital by ambulance, but a sheriff’s office spokesperson says the child passed away.

The accidental shooting happened at 158 Golden Eagle Way early Monday morning in Hendersonville and the girl did not live at this address.

Major Frank Stout, of Henderson County Sheriff’s Office, says “We’re still investigating. We’re not sure of the exact details what led to the accidental discharge from the weapon, so our investigators are working at this time trying to determine what happened.”

We’re told family of the child have been notified, but no other information has been released at this time.

Child accidentally shot in the arm  

(10-26-15)   Erie, Pa.  A child was accidentally shot while sitting in a pickup truck in Millcreek Township.

Millcreek Police were called to a house in the 2000 block of Charleston Avenue just after 12:30 Saturday afternoon.

A 7-year-old boy was reportedly sitting in the back seat of a pick-up parked in the driveway, with his two siblings.

The child reportedly took a handgun from the center console of the truck. At that time, an older sibling pulled the gun away to put back in the council when it discharged, striking the child in the upper right arm.

The child underwent surgery at UPMC Hamot and is listed in stable condition.

The investigation is ongoing.

Deputies: 3 kids injured in accidental shooting in Orlando


ORLANDO -- (10-26-15)
Three children are recovering after deputies say a teen accidentally fired a gun Sunday in Orlando.

The shooting happened Sunday night in the driveway of a home on Alrix Drive.

Deputies said it appears a neighborhood friend in his teens showed up holding a gun. They said he accidentally shot two children.
One child was hit in the foot and another child hit in the hand. A third child went to the hospital with gun-powered burns.
The children range in age from 7 to 13, deputies said.

The injuries are non-life-threatening, deputies said.

Victim named in accidental shooting; loaded gun was on table

(10-26-15)   The 2-year-old victim has been identified as Abigail Newman.

A 911 caller, who said she was the only adult at home with four children, said the victim had been shot in the neck by a shotgun that was left on a table.

"...It was loaded on the table and I didn't know it, and a little girl is shot," she tells the dispatcher. The caller's name was redacted from the 911 recording.

The woman later tells the dispatcher the gun is her husband's and was left on a table by the bed. She says her husband is at work.

The caller says she is holding the child, who is breathing but not very well, and says she is keeping pressure on the wound.

At the beginning of the call, a child tells the dispatcher, "A gun came out and then it shot a little girl!"

The caller said the other children at home are 7, 3 and 4 months old.

Tonopah teen accidently shoots himself with home-made gun

(10-25-15)  Deputies with the Maricopa County Sheriff’s Department are investigating a shooting scene after a young teen accidently shot himself with a home-made gun.

Deputy Joaquin Enriquez says a 13-year-old boy shot himself with a home-made pipe gun that he learned how to make from the internet.

The shooting occurred near 299th Avenue and Roosevelt in Tonopah.

Paramedics airlifted the boy to a local hospital and officials believe he has non-life-threatening injuries, but they are still waiting on an updated condition report.

Cops: Man shot during fight with sister’s boyfriend

(10-25-15 DANBURY — A city man was arrested early Sunday morning after he accidentally shot himself in the arm during a fight with his sister’s boyfriend, police said.
According to police, Savannah Guyton, 23, got into an argument with her boyfriend at her Linron Drive East home around 2 a.m. Sunday. Police said the argument escalated and Guyton bit and hit her boyfriend, who was not identified because he is under 18 years old.
Guyton’s brother, Brian, arrived and began kicking at the door, police said. After the couple let him in, Brian Guyton, 21, pointed a gun at the face of his sister’s boyfriend, police said.
The boyfriend pushed the gun away and the two started fighting, police said. The gun went off during the fight and Brian Guyton was wounded in the left bicep. The sister’s boyfriend then took the gun away from Brian Guyton and placed it on a couch, police said.
Danbury Hospital staff reported the shooting to police around 3:20 a.m. after Savannah Guyton brought her brother there, police said.
The siblings gave accounts that differed from the boyfriend’s statement and other information discovered during the investigation, police said.
Brian Guyton was charged with carrying a pistol without a permit, unlawful discharge of a firearm, interfering with the duties of a police officer, disorderly conduct and threatening. He was being held at the hospital where he needed to undergo shoulder surgery. Bond was set at $150,000.
Savannah Guyton was charged with disorderly conduct, third-degree assault and interfering with the duties of an officer. She was released after posting $1,000 bond.
The sister’s boyfriend was not charged.

Branch County man shoots hand while cleaning gun

(10-25-15)  BATAVIA TWP, Mich. — A Branch County man was airlifted to an Indiana hospital after accidentally shooting himself in the hand.

The Branch County Sheriff’s Office said a 57-year-old man was cleaning his 9mm handgun Sunday afternoon when he accidentally shot himself. He was not aware there was a round in the chamber when the gun fired into his left hand.

Deputies were called to the 300 block of Nash Rd. to assist with first aid. The man was ultimately transported to Parkview Hospital in Ft. Wayne, Indiana by helicopter to treat his significant injuries.

The incident remains under investigation, however deputies don’t believe alcohol was a factor.

Child recovering after mother accidentally shoots him  

(10-25-15)  A child was recovering Friday night after he was accidentally shot by his mother.The incident happened just before 10 p.m. Thursday at a home on Canterbury Drive.Police said the mother was cleaning her gun when it went off and shot her 8-year-old son in the leg.The boy was taken to University Hospital and is expected to be okay.Authorities said the mother would not be charged.

Man Facing Robbery Charges


SPRINGFIELD, Ore. — (10-26-15)  Springfield Police say a man who accidentally shot himself during an armed robbery will face a number of charges.
Police say Casey Carlson broke into an acquaintance’s home Saturday morning around 1:00 a.m.
He then got in a scuffle with the homeowners and accidentally shot himself in the leg.
Police say he’s facing charges of robbery, burglary, and unlawful use of a weapon.
“Right now he’s in the hospital receiving care for a gunshot wound and when he’s released to be able to go into custody, we’ll take him into custody at that point,” said Sgt. Dave Lewis.
Springfield Police say Carlson should be released from the hospital and taken into custody Tuesday.

ACCIDENTAL SHOOTING INJURES HOPKINSVILLE TEEN

(10-25-15)  A Hopkinsville juvenile was treated for non-life threatening injuries Saturday night as the result of an accidental shooting at his residence on Bethel Street.
Hopkinsville Police Sergeant Mike Marzeleck says no foul play is suspected in the shooting, with no investigation taking place into the incident.
Reports indicate a 14 year-old male was cleaning a handgun when it accidentally discharged. When the gun discharged, the teen was shot in the leg. He was taken to Jennie Stuart Medical Center for treatment of non-life threatening injuries. Police are not releasing the victim's name because he is a juvenile and there will be no formal investigation.

Boy dies in accidental shooting, police say 

(10-26-15)  A 3-year-old boy died at the hospital after he was struck in what investigators characterized as an “accidental shooting” at his home in northwest Harris County.

Sheriff’s deputies said the incident happened about 12:30 p.m. Friday inside the two-story home in the 7400 block of Betanna near Elmont.

The boy, his mother, and another young sibling were the only ones in the house at the time.

The mother was in another part of the house when she heard a gunshot, officials said.

She rushed to her two children and found the 3-year-old with a gunshot to the neck or jaw area, deputies said.

The boy, who has not been identified, was flown by LifeFlight air ambulance to Memorial Hermann Hospital at the Texas Medical Center, where he died, officials said.

Sheriff’s detectives said later Friday they have not yet questioned the boy’s mother, who was at the hospital.

The detectives said they were trying to determine how access to the firearm was gained. They did not spell out the type of gun that was fired...

Sheriff’s deputies said the case will likely be presented to a Harris County grand jury once the investigation is complete.
